What is HIPAA?

Defined as the "Health Insurance Portability and Accountability Act”

The HIPAA Privacy Rule provides federal protection for personal health information held by covered entitites and gives patients an array of rights with respect to that information.  At the same time, the Privacy Rule is balanced so that it permits the disclosure of personal health information needed for patient care and other important purposes.  
The Security Rule specifies a series of administrative, Physical, and techinical safeguards for covered entities to use to assure the confidentiality, integrity, and availability of electorinic protected health information.

To request access to your patient health information, you first need to fill out a Release of Information Form for every request of medical information.  Please fax your request to 651-259-4584 or email it to recordsrequests@midwestspine.net  Once your request for records has been received we will contact you with applicable charges and standard rates by law. If you have questions you may call 651-430-3800, ask for Medical Records.

We want to help protect you from identity theft. Therefore, we have adopted an identification process to verify your identity at each visit to be sure your health record is accurate and secure and that any claims filed under your health policy numbers are only for you.
